An improved liquid transfer adapter that provides liquid communication between a vial and a syringe. A liquid transfer has a syringe engagement portion configured to fluidly couple to a syringe and a vial coupling portion of a vial. The vial coupling includes a spike 118 having a spike axis and a tip portion 122 configured to pierce the septum of the vial 102. The tip portion 122 includes a plurality of facets 124 that intersect one another at one or more edges, at least one of the one or more edges being angled with respect to the spike axis. The spike 118 defines a channel 126 in fluid communication with the syringe engaging portion 112 and extending through the spike 118. A channel opening 126 is defined in at least one of the facets 124 and is arranged such that it does not block the edges.
