PROBLEM TO BE SOLVED: To provide a drill guide which is easy to operate, and which, in the event of a positional displacement from a mounted member, can immediately recognize the displacement.SOLUTION: Provided is a drill guide 10 for drilling a hole into which a spinal fixation screw is inserted, including: ring-shaped insertion parts 1 a close fitting part 2 provided with a close fitting surface 2a and through-holes 2b and coupling pieces 3 for coupling the insertion parts and the close fitting part. The close fitting surface of the close fitting part is shaped to allow a male-female relation with a surface shape of a component which includes a predetermined drilling location 11b1. When the close fitting surface is closely fitted, an opening in the close fitting surface is shaped so as to be superposed to encompass the predetermined drilling location such that the predetermined drilling location is centered in the opening, resulting in that the center axis of the insertion part is concentric with a hole to be drilled.
