PROBLEM TO BE SOLVED: To provide a tooth form forming film capable of simply and economically making an artificial crown with accurate occlusion.
SOLUTION: This tooth form forming film 1 is a lid-shaped thin film having such a dimension as covering a single tooth upper face and used when forming the artificial crown for cavity treatment and the like, and is formed with delicate recessed/projecting tooth forms 2 and 3 patterned based on the shape of a real tooth continuously in the inside upper face part and an edge inside part. The recessed/projecting tooth forms are typified and made of silicon or polyethylene into the thickness of 0.1 mm or more. When making the artificial crown, the tooth form forming film 1 is covered thereon and pushed by an opposite tooth to adjust the occlusion before hardening of wax of resin and impresses the delicate shape of the occlusion upper face part and the edge part curve.
