To provide a medical image processing device that can automatically track a marker being kept in a patient's body from a transparent image of a patient receiving radiation in the radiation therapy, and provide a therapy system and a medical image processing program.SOLUTION: A medical image processing device of one embodiment has a first image acquisition unit and a tracking unit. The first image acquisition unit acquires a transparent image of a patient as a first image.
