PROBLEM TO BE SOLVED: To provide an image processing device, an image determination method, and an image determination program capable of accurately evaluating and determining the ratio of tissues in a certain range within a medical image. SOLUTION: A reference tissue brightness calculation unit 120 calculates a reference tissue brightness which is a brightness of a fatty tissue used as a reference for calculating a composition ratio, and an evaluation range brightness calculation unit 130 calculates an evaluation range in which a mammary gland tissue exists. The evaluation range brightness, which is the brightness of each part, is calculated, and the reference brightness ratio calculation unit 140 calculates the ratio of the evaluation tissue brightness to the reference range brightness.
