PROBLEM TO BE SOLVED: To enable autonomous revaluation capable of an improved effect of dementia through rehabilitation training by letting a user independently carry out measuring of dementia and carry out rehabilitation training without imparting any stress to the user regarding a plurality of evaluation indexes for dementia.SOLUTION: Measuring of dementia and rehabilitation training are carried out by using a plurality of game applications corresponding to a core symptom of dementia and the application has a mode selection unit for selecting a measuring mode or a training mode and the measuring mode has functions of: letting a facility user select and execute the game corresponding to the present dementia level of the user by evaluation indexes； calculating a dementia evaluation value from a game execution result and recording it in a user log； and updating the dementia level of the facility user by calculating a dementia level evaluation value from the dementia evaluations value of respective evaluation value indexes. In the training mode, the game selected by the facility user from games corresponding to the present dementia level of the facility user is playable.
