PROBLEM TO BE SOLVED: To provide a clip applier with a counter assembly.SOLUTION: A counter assembly 200 includes a counting mechanism 218, a ratchet mechanism 228, and an actuator 214. The counting mechanism is positioned to rotate within a handle assembly and includes indicia visible through the handle assembly. The ratchet mechanism is rotationally coupled to the counting mechanism and prohibits multidirectional rotation of the counting mechanism. The actuator includes a protrusion 216 projecting therefrom. The actuator is positioned to translate between a proximal position and a distal position, where the protrusion of the actuator engages with the counting mechanism to rotate the counting mechanism, in a first direction, to adjust the indicia of the counting mechanism, which is visible through the handle assembly. In the proximal position, the protrusion of the actuator is disposed proximally to the counting mechanism.
