The cassette (306) has a housing, a gear held by the housing, and a drive shaft driving the gear. The cassette holds the implant such that the gear engages the implant to displace the upper and lower elements relative to one another. The tool body (308) has an inner portion (310) and an outer portion (312) being angled relative to one another. The cassette (306) is mounted to the inner portion (310). The tool body has an outer shaft (326) and an inner shaft rotatably received in the outer shaft (326). The inner shaft includes a flexible joint at the intersection of the inner and outer portions (310,312).
