The implantable lead wire includes a flexible lead body, a plurality of conductor wires, and a plurality of electrodes. The implantable lead also includes a terminal connector assembly connected to the proximal end of the lead body. The terminal connector assembly is sized to be inserted and received in the connector port of the pulse generator header. The terminal connector assembly includes a plurality of terminal ring elements spaced apart in the axial direction, each terminal ring being electrically connected to at least one of the plurality of conductor wires. The plurality of terminal ring elements are separated from each other by an electrically insulating material.
