An implantable medical lead (1) has a distal lead portion (2) with a tubular header (60) and a fixation helix (22) provided in a lumen (61) of the tubular header (60). The fixation helix (22) is connected to a shaft (22) attached to a conductor coil (42). A tubular coupling (28) is connected to the tubular header (60) and is coaxially arranged relative to the shaft (26), with the shaft (26) in its lumen (29). Rotation of the conductor coil (42) causes rotation of the shaft (26) and the fixation helix (22) and longitudinal movement of the fixation helix (22) out of the implantable medical lead (1) by a rotation-to-translation transforming element (21). A friction device (50) is arranged between the shaft (26) and the tubular coupling (28) or between the shaft (26) and the tubular header (60) to oppose rotation of the shaft (26) relative to the tubular header (60) and the tubular coupling (28).
