The apparatus includes a tip portion having a first diameter at a first end and a second diameter at a second end larger than the first diameter, wherein at least a part of the tip portion includes a plurality of pitched threads. A shank is fixedly connected to the second end of the tip portion and has at least a third diameter. A handle is fixedly connected to the shank and arranged distal from the tip portion. The handle has a fourth diameter that is at least greater than the second diameter of the tip portion and the third diameter of the shank.
