Surgical devices and procedures that access tissue and perform microsurgery including capsulotomy of the lens capsule of the eye. The apparatus includes a handpiece having a tip for insertion into the incision. A sliding element is arranged in the handpiece, and a suction cup is mounted on the sliding element. The suction element can be moved in and out of the handpiece by translating the sliding element. A compression mechanism associated with the suction cup and handpiece compresses the suction cup and deploys it through the tip of the handpiece. The suction cup can be expanded into the cutting position inside the anterior chamber. The cutting element attached to the suction cup is used to cut a portion and remove that portion.
