PROBLEM TO BE SOLVED: To improve appropriateness and credibility of a diagnostic name candidate to be provided to a doctor.SOLUTION: A storage processing section 63 of a DB server stores a diagnostic name in a diagnostic name table of a diagnostic name DB 22 after associating the diagnostic name which is input to an input box of a diagnostic name input region with a selection input state of a term selection input region which is a template type operation tool for creating an observation document. When the diagnostic name is already registered in the diagnostic name table, the storage processing section 63 increments the number of input of the diagnostic name. When the diagnostic name is not registered, the storage processing section 63 registers the diagnostic name by adding a new row in the diagnostic name table. A retrieval processing section 64 retrieves and extracts a diagnostic name candidate from the diagnosis name table in response to a retrieval request from a report creation terminal with the selection input state of the term selection input region as a retrieval key and sends a retrieval result to the report creation terminal.
