PROBLEM TO BE SOLVED: To provide a medical information processing apparatus and a medical information processing system for supporting the identification of a patient while considering a change in the body shape of the patient over time. A medical information processing device 10 includes a first storage unit (memory 11), a first acquisition unit (acquisition function 151), and an estimation unit (estimation function 152). The first storage unit stores a past first medical image (medical image DB 21). The first acquisition unit acquires a plurality of first anatomical landmark information from the first medical image. The estimating unit estimates the change in the first anatomical landmark information based on the examination date of the first medical image and the current date, and outputs the estimated result.
