A vial adapter (200) comprises a proximal side having a needle (220) configured to couple with a vial (80)； a cavity (210) extending into the vial adapter (200) and configured to receive a portion of a syringe (100) therein； a gas passage (225) extending between the cavity (210) and the needle (220), wherein the gas passage (225) is configured to fluidly couple with a gas passage (116) of a syringe (100) coupled to the cavity (210)； and a liquid passage (230) extending between the cavity (210) and the needle (220), wherein the liquid passage (230) is configured to fluidly couple with a liquid passage (118) of the syringe (100) coupled to the cavity (210).
