The present invention provides an implant that therapeutically separates the bone of a joint, the implant having two end plates, each end plate having an opening through the end plate, opposite the bone engaging side. The side has at least one inclined surface. The frame is slidably connected to the end plate and the end plates can be moved relative to each other at an angle to the longitudinal axis of the implant while slidably connected to the frame. The actuator screw is rotatably connected to the frame. The carriage forms an empty area that is aligned with the opening in the end plate. The opening in the end plate forms a smooth path from bone to bone of the joint through the carriage.
