The present invention pertains to a method for selecting a wearable device for remotely monitoring a patient with a chronic disease, in particular a COPD patient, the method comprising the steps of:- specifying a plurality of patient groups (A1,...,AN), wherein each of the patient groups (A1,...AN) is defined by at least a set of status parameters of the patients current health status and/or the patients clinical history data- assigning each of the patient groups (A1,...,AN) to a respective set of monitoring parameters (PN,1,...,PN,L)- for each of the patient groups (A1,...,AN), selecting at least one wearable device (14) that includes measurement sensors for remotely measuring at least one of the monitoring parameters (PN,1,..., PN,L) of the respective set of monitoring parameters (PN,1,...,PN,L) of the patient group (A1,...,AN).The invention also includes a method and a system for predicting an acute exacerbation of a health condition of a patient with a chronic disease using the selected wearable device for remote measuring of the monitoring parameters, in order to analyse the measured parameter values and to produce a warning signal if a probability of acute exacerbation exceeds a predetermined value.
