The base data storage part stores a plurality of base data. The image forming part forms a plurality of medical images from each of the base data. The image data storage part stores each medical image. The image formation request management part manages request information for requesting formation of each medical image to the image forming part. According to the request from a client terminal connected through a network, the control part distributes the information on a corresponding examination to the terminal as examination list information. The control part includes the information indicating that an image is being formed in the examination list information.
