The present invention relates to a method and apparatus for verifying a radiation intensity modulator. The apparatus for verifying a radiation intensity modulator according to the present invention comprises: a scanner for scanning a verification target radiation intensity modulator to acquire an image of the radiation intensity modulator, and generating three-dimensional structure information of the verification target radiation intensity modulator based on the acquired image; and a verification system for verifying the radiation intensity modulator based on the three-dimensional structure information of the verification target radiation intensity modulator and original radiation intensity modulator information. The verification system includes: a modulator structure reconfiguring unit for extracting thickness information of the verification target radiation intensity modulator based on the three-dimensional structure information of the verification target radiation intensity modulator; an original modulator structure information reception unit for receiving structure information of an original radiation intensity modulator; a modulator matching unit for matching the verification target radiation intensity modulator with the original radiation intensity modulator based on the thickness information; and a modulator verification unit for verifying the verification target radiation intensity modulator matched with the original radiation intensity modulator.
