Total discectomy and implant insertion into the interbody space are carried out. Then, subcutaneous access is made from the existing skin cut and additional aponeurosis cut is made in close proximity to interspinous space. Resection of the interspinous ligament, caudal portion of upper acantha and cranial portion of lower acantha is carried out, preserving supraspinal ligament.
