PROBLEM TO BE SOLVED: To efficiently generate a synthesis two-dimensional image in which artifact is reduced, from a tomographic image acquired by tomosynthesis imaging in an image processing device, method, and program.SOLUTION: An image acquisition unit 31 acquires a plurality of projection images by photographing a subject M at each of a plurality of radiation source positions. A reconstruction unit 32 reconstructs the plurality of projection images and generates a plurality of tomographic images. A frequency decomposition unit 33 performs frequency decomposition for each of the plurality of tomographic images, and acquires a plurality of band tomographic images indicating a frequency component of each of a plurality of frequency bands for each of the plurality of tomographic images. A two-dimensional image generation unit 34 projects the plurality of band tomographic images for each frequency band and generates band synthesis two-dimensional images.
