A facial mask assembly for use in providing a supply of air at pressure to the entrance of the airways of a person, the mask assembly comprising: a frame a substructure (clip 3234) that is connected to the frame and a cushion (3232) supported by the substructure, the cushion being formed at least in part from foam. A cross-section in at least one region of the cushion has a base side defined by a base surface of the cushion, an outer side defined by an outer surface of the cushion, the outer surface facing away from a centre of the mask, and an inner side defined by an inner surface of the cushion, the inner surface facing toward the centre of the mask, the outer side and the inner side extending away from the base side and intercepting each other at an apex (3600). The base side is engaged to a support surface of the substructure. In use, the facial mask assembly is structured and/or arranged to create a rolling-in effect in the at least one region in which the cushion rolls inwards towards the centre of the mask on application of force to the cushion by headgear straps so that a portion of the outer surface contacts the face of the patient. The rolling-in effect is created by at least one of: (a) an inner peripheral edge of the support surface being outwardly offset from an inner peripheral edge of the base side and (b) the outer side being longer than the inner side.
