The present invention relates to a patient-customized surgical apparatus with a cutting guide which is used to form a surgery area on a femur or a tibia to transplant an implant on the bone. More specifically, the apparatus includes a fixation part and an insertion part. The fixation part includes: a body that is joined with the bone, covers a portion of the bone, and includes a cutting guide insertion part, which is formed on one side of the body and includes an insertion hole penetrated to allow the cutting guide to be inserted therein； the cutting guide which is inserted into one side of the body, guides a cutting tool, inserted to form a cut surface of the bone, and is fixated on the body. The insertion part includes a guide slot through which the cutting tool is inserted and is inserted into the insertion hole to have a gap at certain distance.
