A nerve interface device comprises at least one cuff portion 5, 7; and a first pair of electrodes, 15, 17 mounted on the cuff portion, wherein the cuff portion forms at least part of a passageway for receiving a nerve 13 along a cuff axis 19 passing through the passageway, and wherein the electrodes are spaced apart in the direction of the cuff axis 19. The device may be used in a nerve stimulation system, using a stimulation device to generate an electrical signal. The device may further form part of an implantable system for stimulating and/or monitoring activity of a nerve. The implantable system may be used in a method of modulating activity, or in a method of stimulating and monitoring activity of a nerve. A second pair of electrodes may be mounted on the cuff portion in a ring, concentric to the first pair of electrodes. A physiological sensor and comparison module may also be provided to detect physiological activity and the relationship between the electrical signals and physiological activity.
