The invention relates to a surgical guide device (1) intended to be attached to a bone of a patient for assisting an open wedge osteotomy. This surgical guide device (1) comprises two parts (4, 5) which are joined together by means of removable securing means, namely: - a proximal part (4), comprising a lower border (413) defining at least one portion of the upper edge (21) of a guide window (2), - and a distal part (5) having an upper edge (54) defining at least one portion of the lower edge (22) of the guide window (2). The proximal (4) and distal (5) parts are each provided with a plurality of through holes (42, 56) adapted for receiving members temporarily implantable in said bone (S) and/or adapted to guide a drilling of holes in said bone (S). The arrangement of the through holes (42, 56) of the surgical guide device (1) advantageously takes into account the planned open wedge osteotomy and anticipates the final position of the screw locations of the reported osteosynthesis plate for freezing the two bone fragments.
