A patient tissue includes a primary patient tissue area and an anatomically differentiated bordering secondary patient tissue area. The present invention provides a guide (416, 416') for placing at least one landmark in a scapula having a primary patient tissue area including the glenoid fossa and a secondary patient tissue area including a depression relative to the primary patient tissue at a base of a coracoid of the scapula, the guide comprising:a base (418, 418') having a primary lower base surface (520, 520') contoured to mate with the primary patient tissue area;at least one guiding bore (430, 430') through the base configured to receive and guide a tool for placing the at least one landmark in the patient tissue; andan extended portion (2278) projecting away from and dipping downward from the base to be configured to be nested in the depression, the extended portion having at least one secondary lower base surface located lower than the primary lower base surface and contoured to mate with the secondary patient tissue area in the depression; wherein the primary and secondary lower base surfaces are specific to the patient as a function of preoperative imaging of the patient tissue.
