Devices and methods for treating patients with total or near total occlusion are provided. The device can be positioned within the blood vessel at the treatment site. The occlusion at the treatment site is extended with a catheter. Advance the catheter over the guidewire into the occlusion. One or more of [a] compression or twisting on the guide wire or [b] compression or twisting on the catheter body expands or forms a path through the occlusion.
