A vascular occluder or filter 10 includes a tubular support 12 and a sleeve 30 of occluding or filtering material disposed within the support. The sleeve is twisted in the support creating a constriction 36 which closes the lumen (40, figure 2) of the sleeve. The lumen can nevertheless be opened by a guide wire (54, figure 3) or cannula (52) for over the wire delivery. Once the guide wire or cannula are withdrawn the sleeve will be closed by the action of blood pressure, so reducing the risk of recanalization. The sleeve may be twisted by one or two revolutions or more and is preferably formed of UHMWPE, EPTFE or polyester, and may be woven, knitted or a film. The sleeve is preferably longer than the support (e.g. 5% or 40% longer) and may have its ends wrapped over the ends of the support and secured, e.g. by bonding back to the sleeve. The support is preferably a self-expanding stent.
