Described herein are carrier nanoparticles comprising: (a) a modified liposome having a vesicular structure and comprising lipids conjugated to a polyol polymer via a covalent linkage such that the surface of the liposome presents the polyol polymer a polymer containing a polyol coupled to a polymer containing a boronic acid; (b) a nitrophenylboronic acid-containing polymer that is conjugated to the presented polyol polymer such that the nanoparticle is configured to present the nitrophenylboronic acid-containing polymer to an environment external to the nanoparticle; and (c) an agent exhibiting a chemical or biological activity associated to a target, wherein the agent is contained within the modified liposome.
