PROBLEM TO BE SOLVED: To provide an improved system and method of artificial ankle joint replacement.SOLUTION: A position adjustment device having an instrument holder 134 is fixed in at least two pins projected from the position facing a front surface in the distal end vicinity of a patients tibia. The position adjustment device is adjusted. The position adjustment device is fixed in a first coordinate of the near distal and inside/outside direction with the instrument holder. The tibial distal end is excised by an instrument disposed on the instrument holder while the instrument holder is in the first coordinate of the near distal and inside/outside direction. The instrument is removed from the instrument holder. The tibial temporary insert is disposed on the tibia excised using the instrument holder while the instrument holder is in the first coordinate of the near distal and inside/outside direction.
