A tracheostomy guard comprising a body with a first end 12 configured to couple to a patient interface (1, fig 1) and a second closed end 14 for deflecting patient secretions. The patient secretions are deflected towards openings 15 in the body that allow the secretions to drain from the guard. The second end 14 has an extension of the closed portion into the body to cause further deflection of patient secretions. The guard can be disposable. The guard can taper from the first open end 12 to the second closed end 14. The guard can have a smaller opening positioned away from the second end 14. A larger hole can be positioned to face downwards. The guard can be part of an assembly comprising the patient interface 1 and a tube 6 for the delivery of gases to the patient interface (fig 2). The patient interface 1 can comprise a vent (7, fig 1). In another aspect of the invention, the guard has one of the openings 15 extending nearer the second end 14.
