The invention relates to a scalpel for carpal tunnel surgery by sectioning the transverse carpal ligament, which comprises: - a handle (1)； - an actuator (2) mounted on the handle (1) and capable of moving longitudinally； - a hollow front guide (4) secured to the front end of the handle and which protrudes from the front of said handle (1), said front guide (4) comprising a longitudinal indentation (41) and, at the free end thereof, a hook (42) aligned with the aforementioned longitudinal indentation (41) and suitable for holding the transverse carpal ligament to be sectioned； and - a blade (5) for sectioning the transverse carpal ligament, said blade (5) being connected to the actuator (2) by means of a pin (51), and capable of moving along the longitudinal indentation (41) of the front guide (4), between an inoperative rear position and an operative front cutting position, limited by the hook (42).
