A composition comprising: - 5-20 wt% slurry. buprenorphine in water u polyethylene glycol (PEG) polymer, wherein: - buprenorphine is in the form of particles with an average size of less than 200 microns u composition does not contain polylactide or polyglycolide polymer or smesi.2. A composition according to claim 1, wherein the average particle size of buprenorphine is less than 20 mkm.3. A composition according to claim 2, wherein the particle size of buprenorphine is less than 10 mkm.4. A composition according to any preceding claim, wherein buprenorphine is present in free base form (unprotonated) .5. A composition according to any one of claims 1-3, wherein buprenorphine is present in the protonated salt forme.6. A composition according to claim 1, wherein PEG has a molecular weight of from 1000 to 10000.7. The composition of claim 1 which further comprises a nonionic surfactant selected from the group consisting of Tween 20, Tween 80, poloxamers and fosfolipidov.8. A composition according to claim 7, wherein the surfactant is either Tween 20 or Tween 80.9. A composition according to claim 1, further comprising a pharmaceutically acceptable salts or sugars for regulating isotonicity of the composition and / or a preservative, preferably selected from the group consisting of methylparaben, propylparaben and benzyl spirta.10. A method of treating a patient opioid or lessening pain comprising administering parenteral and extravascular composition according to any one pp.1-9.11. A method according to claim 10 which provides treatment for opioid dependence or pain relief for a period of at least 7 dney.12. A method of treating a patient according to claim 11 which provides treatment for opioid dependence or pain relief for a period not exceeding 30 days1.
