A fluid replacement system comprising: a subsystem (12) for determining an amount of fluid infused (50) into a patient from a first fluid source (16); a pump (18) configured to pump fluid from the first fluid source (16) into the patient; the subsystem (12) also for determining a patient's urine output (52); a user interface (22) including means for setting a desired positive, negative, or zero fluid balance (58) for the patient; and a processing subsystem (20) responsive to a set desired positive, negative, or zero fluid balance (58), and the subsystem (12) and configured to: determine the patient's urine output (52) and the amount of fluid infused (50) into the patient from the first fluid source (16), control the pump (18) to adjust the operation of the pump (18) based on the patient's urine output (52) and the amount of fluid infused (50) into the patient from the first fluid source (16), calculate an actual fluid balance (56) based on the determined urine output (52) and the determined amount of fluid infused (50) into the patient from the first fluid source (16), control the pump (18) based on the actual fluid balance (56) and a set desired negative fluid balance (58) so the actual fluid balance (56) becomes the same as or approximately the same as the desired negative fluid balance (58), when the desired fluid balance (58) is negative and the urine output (52) is less than the set desired negative fluid balance (58) over a first established time interval, control the pump (18) to infuse fluid from the first fluid source (16) into the patient at a preset rate, and when the urine output (52) is greater than the set desired negative fluid balance (58) over a second established time interval, control the pump (18) to infuse fluid from the first fluid source (16) into the patient at a rate which achieves the set desired negative fluid balance (58) based on the urine output (52) during said second established time interval.
