The present invention relates to a medical device comprising an endoscopic appendage attached to a working end of an endoscope. The coupler device provides a flexible working channel extension from the endoscope so that the instrument can be removed from the endoscope at various angles. The device is also provided with a protective cover to reduce the ingress of debris, fluids, bacteria, or other undesirable substances from the working end of the endoscope that can lead to infection.
