The fixation surfaces 66 may be provided on a posterior face 62 of the shell, as well as either on a periphery 64 or at discrete areas on an anterior face. Band-shaped fixation surfaces 66 may be provided on the anterior face of the shell 60 to generally match the angle of pectoralis major or pectoralis minor muscle groups.
