An implantable medical device 10 comprising: a coupling 30； a spacer 40 disposed proximate the coupling； and a plurality of flexible legs 14, where each leg comprises a first end connected to the coupling and a second end 20 remote from the coupling and where the legs extend in a conical configuration from the coupling to a maximum radius of the legs adjacent the second end of the legs, whereby the maximum radius provides a maximum diameter to the device and where the spacer has a deployed radius less than the maximum radius of the legs. The spacer may allow tilting of the device in use. The spacer may comprise at least one curved wire element 42. The coupling may include a device retrieval element 32. The device is described in terms of a vascular filter, where the spacer radius may be calculated with an equation to allow the device to tilt within a vascular vessel, but where the tilt is limited a maximum desired or permitted tilt, ensuring that the coupling is spaced from the wall of the vessel and the coupling is therefore not subject to becoming embedded in the vessel wall due to tissue ingrowth or endothelialisation.
