A method and a device for assisting in C-arm adjustment is proposed, the device being adapted to perform the method comprising the steps of receiving a 2D projection image of an anatomical structure from a C-arm based imaging device, wherein the 2D projection image including a visualization of a reference body has a structure allowing a determination of a 3D orientation of the reference body relative to the anatomical structure based on the 2D projection image, determining the 3D orientation of the reference body, determining an imaging direction based on the 3D orientation of the reference body, determining a body plane based on the 2D projection image, and determining an angle between the viewing direction and the body plane.
