A method of treating a water-deficient dry eye, uveitis, or phacoanaphylactic endophthalmitis in the eye, or balanitis, psoriasis, or atopic dermatitis of the skin, wherein the method comprises the steps of topically administering to the eye or skin a therapeutically effective amount of a cyclosporin A derivative selected from the group consisting of compounds represented by the formula: where R is S-Alk-R, where Alk is an alkylene or alkylenyl bond, R is —N = C (NRR) (NRR) or —NR [(NRR) C = NR], i.e. e. guanidines or -N = C (R) (NRR), i.e. amidines where R-R is H, Alk, Ar or (CH) Ar, where Ar is an aryl group, and n is an integer from 1 to 13, or R and R, or R and R, or R and R, or R and R, or R and R, or R and R together may be - (CH) -, where x is an integer from 2 to 5, and R is selected from the group consisting of hydroxyl, lower alkyl and substituted by hydroxyl lower alkyl. 2. The method of claim 1, wherein R1 is methylene or a C-Cpolymethylene bond. The method of claim 1, wherein R4 is a C-Calkenylenyl bond. The method of claim 1, wherein R is —N = C (NRR) (NRR) or —NRC (NR) (C = NR), where R — R is H, Alk, Ar or (CH) Ar, where Ar is is an aryl group and n is an integer from 1 to 13, or R and R, or R and R, or R and R, or R and R together can be —CH — CH— or —CH — CH — CH — .5. The method of claim 1, wherein said cyclosporin A derivative is a 3-substituted diaminoiminoalkylthiocyclosporin A.6 derivative.
