The appliance 1 comprises an outer wall 7 and an inner wall 9, and a web 11 extending between the inner and the outer walls. The inner and outer walls and the web define upper and lower channels within which the respective arches of a patient can be received. The inner and outer walls 9 and 7 form a mounting arrangement for mounting the appliance on the upper and lower arches 85 and 87 of the patient. The outer wall 7 10 has an upper portion which is offset relative to the lower portion and the upper portion 7 forms an outer spacing formation 5 for holding the buccal mucosa away from the patients upper arch 85 so that it does not inhibit development of the upper arch 86.
