An intramedullary based & omni-positionable cutting guide instrumentation forfemoral epiphysis resection for kneeprosthesis placement comprising: an intramedullary placement guide (1,27) in amedullary cavity of a femoral epiphysis having ananterior/posterior adjustment mechanism (33)； posterior rotational referencingsizing jig (8) for fastening to the anterior/posterioradjustment mechanism (33) and configured for referencing anterior (10) andposterior (11, 18) surfaces of the femoral epiphysis toadjust the anterior/posterior offset of the anterior/posterior adjustmentmechanism (33)； and a punch card (20) for subsequent fasteningto the anterior/posterior adjustment mechanism (33) and comprising a pluralityof position referencing apertures (21a- 21d) forreceipt of at least one locking pin (23) of a cutting block guide (5) to allowthe anterior/posterior positioning of the cutting block guide(5) at a first position (e.g. fig 10) for making a first resection (e.g. 24)and at a second position (e.g. fig 11) for the purposes ofmaking a second resection (e.g. 37).
