PROBLEM TO BE SOLVED: To implement a structure of a surgical cable sleeve and a sleeve holding instrument that makes it possible to safely, securely, and easily provide the surgical cable sleeve at the time of an operation using a surgical cable.SOLUTION: A surgical cable sleeve system comprises: a surgical cable sleeve 10 including a sleeve main body part 11 comprising a cable insertion hole 11a into which a surgical cable is to be inserted and a sleeve holding instrument 20 that has sleeve mounting parts 21a, 21d, 22a, 22b, 22c detachably connected to the sleeve main body part 11 at the tip of the instrument, extends from the tip in a direction orthogonal to an axis line of the cable insertion hole 11a, and has a base end part on the side opposite to the sleeve mounting parts. The sleeve mounting parts include: sleeve restriction fitting parts 21a, 21d, 22a for fitting to the sleeve main body part 11 to restrict the sleeve main body part 11s posture and sleeve holding parts 22b, 22c for holding the sleeve main body part 11 in a state in which the sleeve main body part 11 is fitted to the sleeve restriction fitting parts.
