A surgical instrument has a shaft with a proximal end and a blunt enclosed distal end with an optically transparent window 702. A viewing instrument channel is provided within the shaft which accommodates a viewing instrument 802 with a viewing end inserted from the proximal end and movable between first and second positions. When the viewing instrument is in the first position (Fig 13) within the shaft the viewing end is on the distal side of a working area within which resection can occur and proximate to the optically transparent window of the distal and so as to provide an unobstructed view external of the blunt enclosed distal end. When the viewing instrument is in the second position (Fig 14) within the shaft the viewing end is on the proximal side of the working area and provides a view of the working area.
