To provide a pharmaceutical composition or the like used for the treatment or prevention of angiogenic diseases. A combination of a monoclonal antibody or antigen-binding fragment thereof having the following characteristics (i) to (v) and a VEGF antagonist, or a pharmaceutical composition comprising the antibody or antigen-binding fragment thereof and a VEGF antagonist: (i) Binds to ROBO4 protein with a Kd value of 1 × 10 −8 M or less； (ii) specifically binds to human ROBO4 protein shown in SEQ ID NO: 2 in the sequence listing； (iii) in vitro Suppress or inhibit vascular endothelial cell migration in the presence； (iv) suppress or inhibit angiogenesis in vivo； (v) a chimeric antibody, a humanized antibody.血管新生疾患の治療又は予防に使用される医薬組成物等を提供すること。 下記（ｉ）乃至（ｖ）の特徴を有するモノクローナル抗体もしくはその抗原結合断片、及び、ＶＥＧＦアンタゴニストの組合せ、又は、該抗体もしくはその抗原結合断片、及び、ＶＥＧＦアンタゴニストを含む医薬組成物：（ｉ）１×１０－８Ｍ以下のＫｄ値でＲＯＢＯ４蛋白質に結合する；（ｉｉ）配列表の配列番号２に示されるヒトＲＯＢＯ４蛋白質に特異的に結合する；（ｉｉｉ）イン・ビトロにおいて、クロスリンク抗体非存在下で血管内皮細胞遊走を抑制又は阻害する；（ｉｖ）イン・ビボにおいて、血管新生を抑制又は阻害する；（ｖ）キメラ抗体、ヒト化抗体である。
