An end face of working portion is connected to circumferential wall (8). The outer diameter of end face is larger than outer diameter of shaft. The end face formed opening (9), is connected to a liquid conduit, so that the liquid conduit leaks liquid from the opening. The circumferential wall is formed with a rounded outer periphery, so as to mechanically separate the two different tissues from each other.
