A dilator for sheathless access to a blood vessel of a transradial catheterization patient. A dilator (10) for sheathless access to a patient's blood vessel includes a distal portion (14) having a maximum outer diameter extending from a distal end (12) to a guidewire exit port (16) and a proximal portion from the guidewire exit port. It has a proximal portion 18 with a reduced profile that extends to an end 20 and a lumen 22 that extends between the distal end and the guidewire exit port. A first guidewire can be placed within the blood vessel and the dilator can be advanced over the guidewire. The guide catheter can then be advanced along the dilator.
