An arterial access sheath 220 includes an elongated body sized and shaped for transcervical introduction into the common carotid artery at the neck access position, and an elongated body adjacent the elongated body. A proximal opening in the proximal region and an inner lumen having a distal opening in the distal region of the elongate body. When the elongate body is placed in the common carotid artery, the internal lumen provides a passageway for introducing the interventional device into the common carotid artery. The elongate body has a proximal portion and a distal most portion that is more flexible than the proximal portion. The ratio of the total length of the most distal portion to the total length of the sheath body is 1/10 to 1/2 of the total length of the sheath body.
