Disclosed is a method for calculating a instantaneous wave-free ratio and a diastolic pressure ratio during angiography on the basis of angiography images, comprising: measuring the pressure P a of the coronary orifice during diastole (S01); obtaining the two-dimensional vascular diameter and length of the blood vessel via angiography images, generating a three-dimensional blood vessel mesh model from the two angiography images, and obtaining the three-dimensional vascular diameter and length of the blood vessel (S02); measuring the time that the blood containing a contrast medium goes from a start point to an end point of a specified length of blood vessel during diastole, and calculating a blood flow velocity V 1 according to the time and the three-dimensional length of the blood vessel (S03); with V 1 as the coronary orifice flow velocity, calculating a pressure drop ΔP from the coronary orifice to the distal end of a coronary stenosis; with the average intra-coronary pressure at the distal end of the stenosis P d = P a-ΔP, calculating the instantaneous wave-free ratio and the diastolic pressure ratio during angiography (S04).
