PROBLEM TO BE SOLVED: To provide a sustained systemically acting external preparation having salt-type medical drug which is absorbed from skin as a principal agent.SOLUTION: A sustained systemically acting external preparation contains a core-shell structure in which a core part contains a salt-type medical drug with a molecular weight of a medical drug area of less than 400, and a shell part contains at least one surfactant which is selected from sucrose fatty acid ester, glycerin fatty acid ester, polyoxyethylene glycerin fatty acid ester, sorbitan fatty acid ester, polyoxyethylene sorbitol fatty acid ester, polyoxyethylene castor oil and hardened castor oil, and in which a weighted average value of HLB value is 10 or less.
