The planar oral care patch comprises a first split stretching along a first direction in a central region of the planar oral care patch so that the planar oral care patch can be folded as a three-dimensional shape. An oral care patch tape having a plurality of the planar oral care patch is provided. A method of using the planar oral care patch, a method of treating an oral disease and a method of prophylactically treating an oral disease by using the planar oral care patch are also provided.
